Talk to your Department's PG Studies director, or that PG lead in your Faculty, about ensuring there's a review of your PhD prior to submission, so there's an outside eye on it. This is what we have 2nd supervisors for, or even a supervisory committee (that's the US convention). Even your supervisor might be too close to it now.
Distance learning PhD. As PhDs are based primarily on independent research rather than time spent in lectures and seminars, distance learning is a viable route for many students. If you have family or work commitments, or are an international student, this gives you the chance to undertake a PhD without having to live close to your chosen.

It's exceptionally rare. Generally to fail outright you'd have to falsify data or otherwise catastrophically screw up. The normal scenario is you get a Masters of some kind (M.Phil at my university), but generally you'd have been bumped from the PhD programme before defence if you were really doing that badly.
